What you need for this position:


- AA in a related field required; B.S. in a related field preferred

- Experience working with HIV-related illnesses, substance abuse, groups and children required

- Experience with field work, home visits, case finding and outreach preferred

- Ability to communicate clearly in both oral and written forms required

- Ability to work effectively in teams required

- Bi-lingual English/Spanish preferred


What you'll be doing:


- Providing assistance to case manager/team leader and provides direct case work activities including but not limited to assessments, information gathering, service plan implementation and advocacy and referrals

- A minimum of 25 hours of direct casework is required per week

- Coordinating all assessment, develops case plans and referrals for services

- Keeping daily time sheets and statistics

- Discussing program and services with participants from intake through discharge

- Establishing and maintaining case records

- Obtaining all necessary records from other agencies involved with our COBRA clients

- Meeting with participants as needed for follow-up and recommendation

- Making referrals for services

- Conducting case conferences

- Conducting home visits, case finding and outreach routinely in order to maintain caseload

- Participating in team meetings & staff trainings

- Performing other related and unrelated duties as directed by supervision
